What Kind of Support is Right for You?

 

Mesa County Passages offers several programs and experiences to help you transition from foster care to independent adulthood.

  • Coaching services and life skills education

  • Weekly coaching focused on YOUR goals & desires.

  • Housing vouchers and support to help you obtain independent housing.  

  • DHS Transition Services
    While you are gaining independence your living situation could be an independent living program, transitional living program, or a family foster home. You will maintain all of your rights as an adult while partnering with Mesa County DHS for a few months or a few years. 

All programs are voluntary, directed by you, and confidential. We respect and honor your choices.

You do not have to be eligible for the Passages Program to become a Passages Member. Complete a referral to learn more.

After you complete the referral form, a Passages team member will reply within three business days. If you qualify, we will let you know the specific programs you are eligible for. If you are being abused or neglected, please call our hotline directly at (970) 242-1211.

Need Help with Housing?

The Passages Team can help you to apply for the Foster Youth to Independence Housing Voucher, which provides 36 months of rental assistance.

  • HomewardBound of the Grand Valley is a non-profit organization that provides critical housing, compassionate services, and safe shelter for individuals and families. https://homewardboundgv.org

  • The Hilltop Family Adolescent Partnership provides case management and funding support for families in Mesa County. https://www.htop.org/fap

  • The Joseph Center supports families at the intersection of homelessness and parenting. https://josephcentergj.com

  • I Matter offers three free mental health sessions to Colorado children and teens between 12 and 18 years old. https://imattercolorado.org

  • The world's largest suicide prevention and crisis intervention organization for LGBTQ young people. They provide information & support to LGBTQ

  • Healthy Mesa County is a website listing resources, activities, and tools for Mesa County residents. https://healthymesacounty.org/

  • Food Bank of the Rockies operates mobile food pantries and distribution centers in Mesa County. No eligibility requirements. https://www.foodbankrockies.org/find-food

  • Mesa County Workforce Center can help you find a job and learn about training opportunities, including apprenticeships, GED classes, and childcare assistance. The Career Development Program at the Mesa County Workforce Center, through WIOA grant funding, can assist with paid internship placements, paid on-the-job training placements, paid GED study time, as well as tuition assistance for eligible training programs in high-demand occupations. https://mcwfc.us/job-seekers/career-development/
