  • The Chafee Foster Care Independence Program provides ongoing support to youth and young adults ages 14-23 adults as they overcome the hurdles of living independently, including voluntary coaching services and life skills support.

  • Pathways is a specialized program that Chafee participants can opt-in or opt-out of consisting of weekly coaching while highlighting youth voice.

  • The Foster Youth to Independence housing voucher (FYI) is for youth and young adults 18-23 who are not in DHS care. Valid for three years, it is designed to support young adults in obtaining independent housing.

  • The Foster Youth in Transition (YiT) Program is designed to assist older youth in transition to adulthood. Services may include assistance with:

    Accessing education or vocational training

    Enrolling in Medicaid

    Securing and maintaining employment and housing.

    Opening bank accounts and financial education.

    Obtaining a driver's license, vital documents, records, and information about relatives and siblings.

    Designing an individual roadmap to adulthood life.

    The youth in this program voluntarily re-enter DHS care and placement, which may consist of an independent living program, transitional living program, or a family foster home. They choose the services and support they receive and may receive help until age 21.

    Any youth that was in DHS or DYS custody between 16-18, who is now over 18 and at risk of homelessness, can self-refer.

  • HomewardBound of the Grand Valley is a non-profit organization that provides critical housing, compassionate services, and safe shelter for individuals and families. https://homewardboundgv.org

  • The Hilltop Family Adolescent Partnership provides case management and funding support for families in Mesa County. https://www.htop.org/fap

  • The Joseph Center supports families at the intersection of homelessness and parenting. https://josephcentergj.com

  • I Matter offers three free mental health sessions to Colorado children and teens between 12 and 18 years old. https://imattercolorado.org
