What is Health First Colorado?

 

Health First Colorado is Colorado's Medicaid program and offers free or low-cost health care for eligible children and adults. This includes doctor visits, hospital stays, prescription drugs, dental care, and more. Health First Colorado makes sure that people who qualify can get the care they need.

You might qualify for Health First Colorado (Medicaid) if you have a low income. This includes your babies or children if you are a parent.


 

What is Health First Colorado?

 

If you're pregnant or have children, but your income is too high to qualify for Health First Colorado, you might qualify for Child Health Plan Plus (CHP+).

How Do I Know if I Qualify for Medicaid or CHP+?

 

You might be able to get Medicaid or CHP+ if:

  • You don't have a lot of money coming in. We look at your income and the number of people in your family. If you are pregnant, it counts as 2 people. Every person and every family is unique.

  • You're working or in school, or you're looking for work.

  • Your children may be able to get Medicaid or CHP+ even if you can't, as long as you meet a few other requirements. Please apply as soon as you can.

How to Apply for Medicaid and CHP+

 

Applying for Health First Colorado (Medicaid) and Child Health Plan Plus (CHP+) is simple. There are two ways to apply:

Online:

Complete an application on the Colorado PEAK website.

Over the Phone:

Reach out to the Mesa County Dept. of Human Services Call Center at (970) 256-2434 for help with:

  • Submitting an application

  • Eligibility questions

  • Checking your application status

  • Scheduling an interview

  • Paperwork questions

  • Reporting changes in your household or income

  • Medicaid and other medical programs

What to Expect:

 

When you apply:

You will provide information about your income and household. Then, you will find out whether you qualify for Health First Colorado (Medicaid) or Child Health Plan Plus (CHP+).

We will NOT ask you for:

  • Bank account statements

  • Phone or utility bills

  • Proof of your address


 

But What if You Don’t Qualify for Health First Colorado (Medicaid) or CHP+?

 

If you don’t qualify for Health First Colorado or CHP+, you might still get help to lower the cost of private health insurance through Connect for Health Colorado.

Connect for Health Colorado is the official health insurance marketplace for Colorado residents. It provides financial assistance to make health coverage more affordable.

Financial assistance provided by Connect for Health Colorado can help lower your monthly insurance bill or the amount of money you have to pay when you get medical care.

Colorado PEAK is an online service where you can apply for benefits such as Health First Colorado (Medicaid) and Child Health Plan Plus (CHP+). It allows you to check your eligibility, submit applications, and manage your benefits all in one place. Colorado PEAK makes it easy to access the health care and financial support you need.

You can also apply for food assistance, cash assistance, and other benefits online using Colorado PEAK.

The Healthy Teens program helps young people make good choices about their health. They offer private services for teens, including info about your body, birth control, pregnancy, and ways to stay healthy. They also answer questions about relationships and bust common myths. If you're under 20, let them know when you call so they can see you quickly. Everything you talk about stays between you and them.

WIC (Women, Infants, and Children)

 

WIC (Women, Infants, and Children) provides healthy food and nutrition education to moms and kids. If you're pregnant, just had a baby, or have kids under 5, WIC can help. They give you healthy food, teach you about good nutrition, help with breastfeeding, and connect you with other health services. WIC wants to help you and your family stay healthy.

Nurse-Family Partnership

 

The Nurse-Family Partnership program helps first-time moms by providing support during pregnancy and until the baby is two years old. A specially trained nurse will visit you at home or meet you at a comfortable place to answer questions and provide guidance on pregnancy and parenting.

Grand Valley Connects

 

Grand Valley Connects helps people in Mesa County access different resources and services. A Resource Navigator can help with behavioral health, childcare, education programs, job services, food assistance, medical health coverage, and more.

To connect with a Resource Navigator, fill out the referral form online or call 970-683-2663. Your Resource Navigator will talk with you about your needs and create a plan. 

Immediate help is available for crises. You can refer yourself or even refer someone else.
